By submitting an application, you agree to the following terms and conditions:

• I agree that all the information I have provided on this pet insurance application is correct, and that this application will not result in coverage.
• I understand that the payment information that I have provided will be used to collect premium and fees in the amount, and at the frequency I have chosen on the application
• By submitting this application, I am providing consent for Pets Best to provide required documents to me electronically. This consent for electronic delivery covers all documents that we are required to give you for this and every one of your accounts with us and is effective until withdrawn by you.

Fraud Warning

Any person who, with intent to defraud or knowing that he is facilitating a fraud against an insurer, submits an application or files a claim containing a false or deceptive statement is guilty of insurance fraud.
